Requirements for ALL Drivers License including Out of State, or Florida ID cards.

• Original Operator Drivers License & birth certificate-Two forms of proof of residential address, as described in information that follows. The names MUST match exactly. 
• Your out of state driver license (if coming from out of State).
• Vision test, with or without corrective lenses, to meet acuity of 20/40.
• Hearing test.
• Social Security Card is required
• If name changing from birth certificate to present name-- You MUST show connecting documents. (example married name bring in-- ALL Certified Marriage Certificates)

All persons obtaining an Original & Learners License

• Original Birth Certificate from Vital Statistics or Current Passport (No Photo Copies). 
• Traffic Law and Substance Abuse Education Course (TLSAE). This course is provided by outside companies, and can be located by looking in the phone book yellow pages, under Driver Instruction.
• Under age 18 need a legal guardian to sign for his or her license upon receiving.
• A written test is required, which includes 50 questions of road rules and road signs. The test may be waived for Driver Education Students, with proper documentation from their school.
• Social Security Card is required
• Two  proofs of Residential address.
• **If name have been changed since Birth need a Certified copy of Marriage Licenses.**

 Original & Renewal Commercial Drivers License (CDL)

• Original Birth Certificate, Social Security Card & TWO proofs of residential address. The names MUST match exactly. Commercial Drivers License Operators are required to present a Social Security Card for issuance of a Florida CDL.
• Vision test, with or without corrective lenses to meet acuity of 20/40.
• Hearing test.
• All tests and endorsements are reciprocated EXCEPT Hazardous materials. (Hazardous endorsement is NOT done at this location)
• Medical Card Required.
